How to Issue a Practitioner Referral Request to Patients for Circadian Phase Testing.

For registered providers, you can use your Practitioner Referral ID# to send an assessment referral request to an individual’s e-mail address. Previous order requests can be found in the Referral History under your account profile.

Referral Request Step-by-Step Outline:

1. Build a Shopping Cart with the required assessment or assessments. You do not need to be logged into an account.
2. On the “Cart” page click on “Send a Referral Request” button (top left).
3. Enter your Practitioner Referral ID#, Patient Email, and Patient Phone Number.
4. Click Submit.

* You can use the same cart or build a new cart to send additional referrals to different patients.
